NATIONAL PRIZES

The Grand Prize will be awarded to the individual (Adult and/or Youth) who has individually raised the most amount of money nationwide.

Adult Individual Category: First, second and third prizes in the Adult Individual Category will be awarded to the adult, 18 or older, who has individually raised the most money nationwide in descending monetary order.

Youth Category: First, second and third prizes in the Youth Individual Category will be awarded to the youth (under 18 years of age at the time of the Hoop) who has individually raised the most money nationwide in descending monetary order.

No substitutes or deviation from the age category will be permitted nationally. Prizes are awarded to the top individual in adult and youth categories only.

GRAND PRIZE

Holland America 7-Night Caribbean/Mexico/Alaska Cruise in an Outside Cabin

The winner will enjoy an outside cabin (Category F) featuring between 185-197 square feet for a 7-night Caribbean, Mexico or Alaska (May and September) sailing valid through December 2008 and is subject to availability. Holland America is an Award-winning cruise line which was selected as a prestigious member of Conde Nast's 2006 Gold List ahead of popular cruise lines such as Princess, Royal Caribbean, Norwegian Caribbean, Cunard, Costa and more. Enjoy your outside cabin for two for a seven-night cruise. Air taxes and security fees are the responsibility of the winning bidder. Port charges and government fees are included.

Alaska-The winning bidder can travel on the Veendam, Zaandam, Ryndam or Statendam in May or September.
Caribbean/Mexico-The winning bidder can travel on the Veendam, Westerdam, Zuiderdam or Oosterdam.

*Upgrades change the available ships. Choice of sailing dates will be accommodated on a space available basis and will be confirmed at time of request. Blackout dates may apply (holiday sailings are excluded). Third and fourth persons are permitted in the same cabin and will be charged the per person tariff per applicable brochure. Cancellation of a cruise after the cruise has been confirmed will result in the forfeiture of the charity cruise certificate.

This package includes round-trip coach class service for two from any American Airlines destination in the 48 contiguous United States to anywhere American Airlines flies in the 48 contiguous United States subject to availability. A minimum of 14 days advance booking is required as is a Saturday night stay. Miles cannot be utilized to upgrade these coach class certificates to a higher level of service. All reservations are subject to availability.

Any airport departure or taxes (if charged) are the responsibility of the winning bidder. Ground transportation is not included in this package.

A WORD ABOUT HUNTINGTON'S DISEASE

Huntington's Disease (HD) is an inherited, degenerative brain disorder
that results in the progressive loss of control of both the body and the mind.
There is presently no cure or effective treatment for HD. The New Jersey
Chapter of the Huntington 's Disease Society of America (HDSA) is the only
volunteer organization in the State dedicated to assisting affected families
and supporting research to find a cure.

( via Route 287 South) Leave Route 287 South at Route 529 South (Stelton Rd.) Exit.
Turn right onto Metlars Lane. Bear Left onto Suttons Lane. Turn right onto Hospital Road.

Directions from Route 287 ( North or South )
Take Route 10 East Exit ( proceed 2 miles )
1st light - Jefferson Road ( Hanover Municipal Building )
2nd light - Pine Plaza /
3rd light - Whippany Road ( Shell & Exxon Stations )
4th light - Troy Hills Road ( Molly Malones / Whippany Diner)
5th light - Algonquin Parkway ( PC Richards / Suburban Propane Take jughandle across Route 10 onto Algonquin Parkway
Hoop Heaven is 1/2 mile down Algonquin Parkway on left hand side

Directions from Route 280 ( East or West )
Take the Eisenhower Parkway Exit ( 4A ) Come off exit and proceed straight. Look for Houlihan's, Grand Union, Blockbuster Video, Olive
Garden at
3rd light. The 4th traffic light is Route 10. Make right onto Route 10 West.
Go 7 traffic lights. Algonquin Parkway is the 7th traffic light. Pass Old Navy,
Toys R Us, Just for Feet, Tops, Pathmark, Wesco, Warnock Auto,
SONY Theatre, Ramada Inn, Ridgedale Avenue, Novartis, Anchor Golfland, and Golds Gym. Algonquin Parkway is the traffic light just past
Golds Gym. Make a right and proceed 1/2 mile. Hoop Heaven is on left hand side.

From Newark Airport Exit Airport.
Follow overhead signs to Route 78 Westbound. Take Route 78 West, use local lanes, to NJ Route 24 West. Exit Route 24 at Exit 1B
Whippany Road North. Proceed to Route 10. Make right onto Route 10 East. Algonquin Parkway is one mile eastbound on Route 10.

From NJ Turnpike South
Get off at exit 14. Follow overhead signs to Route 78 Westbound. Take Route 78 West, use local lanes, to NJ Route 24 West. Exit Route
24 at Exit 1B Whippany Road North. Proceed to Route 10. Make right onto Route 10 East. Algonquin Parkway is one mile eastbound on
Route 10.

From NJ Turnpike North
Get off at exit 10. Follow overhead signs to Route 287 North. Proceed on 287 for 35 miles until
Exit 39. Follow overhead signs to Route 10 East, Whippany. Algonquin Parkway is two miles eastbound on Route 10.

From Garden State Parkway South
Take GSP to Exit 142A. Take Route 78 West, use local lanes, to Route 24 West. Exit Route 24 at Exit 1B, Whippany Road North.
Proceed to Rt 10. Make right onto Route 10 East. Algonquin Parkway is one mile east

From Garden State Parkway North
Take GSP to Union toll barrier. Proceed to extreme right toll lane. After toll, follow overhead signs to Route 78 West. Proceed for 1 mile to
first exit which is exit 54 Hillside/Irvington. Bear right off the exit ramp. Proceed over Route 78. Make right onto entrance ramp for Route
78 West. Take Route 78 West, use local lanes, to NJ Route 24 West. Stay on Rt. 24 for approx 10 miles. Exit Route 24 at Exit 1B
Whippany Road North. Proceed to Rt 10. Make right onto Rt10 East. Algonquin Parkway is one mile eastbound on Rt 10.
